The Studio loves getting new students! Our teachers have a real enthusiasm for music and want to share it.
We do ask new students to fill out our “New Student Form.” The form not only gives us an idea of the student’s current skill level, but also helps us pair the student with the most appropriate teacher and schedule a time slot agreeable to all. The new student form is below in two formats, please fill it out and email to dori@dorimusicstudio.com or bring to the studio during business hours.
If you have a word processor, you can open the form in the Microsoft Word format, fill it in on your computer, save it, and attach it to an e-mail, and mail it back to us at dorimusicstudio@gmail.com.
We have also provided the form in a pdf format. You can print this form, fill it in by hand and bring it into the studio or drop it in the U.S. mail.
