The Studio loves getting new students! Our teachers have a real enthusiasm for music and want to share it.
We do ask new students to fill out our “New Student Form.” The form not only gives us an idea of the student’s current skill level, but also helps us pair the student with the most appropriate teacher and schedule a time slot agreeable to all. The new student form is below in two formats, please fill it out and email to dorimusicstudio@gmail.com or bring to the studio during business hours.

It’s a PDF, but you can use it one of two ways:
- Download it, print to your printer, and fill it out by hand or with typewriter. You can then bring it into the studio or drop it in the U.S. mail. You can also scan it in and email it.
- Download it, open it up in a browser (it’s editable!), edit it, and then save your edited copy. You can then email the saved copy or print it out and bring it into the studio or drop it in the U.S. mail.
